Enrollment and Registration

Effective, Monday, April 6, 2020, Evergreen Elementary School District will be accepting new student registrations via email through the following steps:

  1. Go to the School Locator and enter your house number and street name to locate your home school of residence.
  2. Determine your child’s grade level according to birthdate.
  3. Print and complete the appropriate grade level registration packet below.
  4. Gather and then scan or take a picture of these additional required documents:
    • Three to four proofs of residency (see registration packet)
    • An image of an original birth certificate or passport for the child
    • Proof of immunizations (see registration packet)
    • A completed and signed image of the HLS (Home Language Survey) form
  5. Email all registration forms and required documents to the school secretary at your home school of residence. This can be found on the front page or in the staff directory of your home school's website.
  6. Spanish Dual Language Program Application - After completing steps 1-5, see instructions in the Spanish Dual Language Program section below.

Students residing in the Evergreen School District (ESD) are assigned an elementary and a middle school based on their home address. Enrollment at the school of residence is on a first come, first served basis. Every attempt will be made to place new students at their school of residence. However, the district cannot guarantee such placement and reserves the right to overflow students to other schools within ESD as necessary when a grade-level enrollment capacity is reached. Any student overflowed to a school other than their school of residence will be provided transportation from their school of residence and back. Overflowed students will return to their school of residence the following school year or as space becomes available.


Registration Packets by Grade

If you are unable to print a registration packet, please email info@eesd.org and request a packet be mailed to you. Please include the address you want to receive the packet along with the grade level and preferred language packet you want.

Transitional Kindergarten Registration Requirements

1. The child must turn 5 years old between September 2 and December 2 (of the current year).

2. Parents and child reside in the Evergreen School District attendance area. NOTE: Registration packets must include proof of residence.

3. An original birth certificate with seal or other legal document showing date of birth must be presented to school personnel.

4. Record of UP-TO-DATE IMMUNIZATIONS must be presented to school personnel. If immunizations are not up to date, the registration will not be considered complete. Schools in California enforce a “no shots, no school” policy. The child WILL NOT be allowed to attend school if the immunization verification has not been received before the first day of school.

5. All transitional kindergarten students must be evaluated for risk factors for TB as part of their routine medical “assessment.” A Tuberculin Skin Test (TST ) or other TB test (IGRA blood test) approved by the Centers for Disease Control and Prevention will be ordered by the healthcare provider (HCP) if deemed necessary based on the TB risk factor assessment. The Santa Clara County Public Health Department TB Symptoms Review and Risk Factor Assessment Form which will be completed by the HCP will be returned to your student’s school, along with the registration packet.

The student’s HCP will give parent/guardian documentation stating one of the following within 1 year of registration:

a. Student’s risk assessment was negative. No TB testing is required.
b. Student’s risk assessment was positive with attached TB screening test (TST/IGRA) and CXR result. Student is free of infectious tuberculosis.

Students with TST/IGRA test results within 1 year of registration may submit their results with their registration packets and do NOT need to obtain the Assessment of Risk Factors from their HCP.

6. Students are required to have a dental check-up by May 31 of their kindergarten year. Oral evaluations completed within 12 months prior to school entry also meet this requirement.

7. Each child should receive a physical examination 18 months prior to entrance into the first grade or within 90 days after first grade entrance. Physicals must be given after March 1 (of the current year).

The Vaccines for Children (VFC) helps families by providing vaccines at no cost to providers who serve eligible children from birth through 18 years of age. Parents who have no health insurance or if they have health insurance but no vaccine coverage they can contact:

IMMUNIZATIONS

Polio

4 doses

Three doses of polio vaccine meet the requirement if one dose was give on or after the 4th birthday.

DPT/TD



5 doses

Four doses of DTap meet the requirement if at least one dose was given on or after the 4th birthday.

MMR

(Measles, Mumps, Rubella)


2 doses


Must be given on or after the first birthday. Child who has just received first dose of MMR may enter kindergarten if they receive a 2nd dose 1 to 3 months after the first dose.


TB Risk Assessment


All students entering school for the first time (TK or Kindergarten) or transferring into Santa Clara County MUST present written evidence of having had a TB Skin Test (TST/Mantoux/PPD) completed in the United States within 12 months of registration OR a Santa Clara County Risk Assessment form signed by a physician or clinic. The TST must include the date given, the date read, and results of the test with signature of the physician or clinic; OR an IGRA blood test with results

Hepatitis B

(chickenpox)

2 doses of Varicella vaccination OR documentation of permanent medical immunity.

Residency Verification

Proof of residence is required for registration at ESD schools. Acceptable Proof of Residency Requirements are listed on the D-90G Residency Verification Declaration, which is included in the online registration packets (2020-2021 Registration Information and Forms link above). A school which has been designated as an Impacted School require additional proof of residency requirements. Carolyn Clark and Matsumoto elementary schools are designated as Impacted Schools. These two schools require 4 proof of residency.

School Choice Transfer Request

The parent/guardian may request that a child attends a school other than their home school within ESD through a School Choice Transfer Request. Information regarding the program is located on the website in the School Choice section.

Some highlights of the School Choice Transfer Request process include:

  1. Child must be registered at their designated home school.
  2. Transfer requests are approved based on a lottery and on space availability for School Choice Transfer Requests at the requested school.
  3. Parent/guardian is responsible for the transportation of students approved for a School Choice Transfer Request.
  4. Approval is for a specific school.
  5. Students who are not approved for a School Choice Transfer Request will attend their home school.

Spanish Dual Language Program Application

At Evergreen School District we offer a Spanish Dual Language Program located at Holly Oak Elementary School. Only kindergarten and transitional kindergarten students may apply for this program. Learn more about our Dual Language Program.

Registration Information
Enrollment for the Dual Immersion Program begins on February 3, 2020. Please follow these steps to apply:

  1. Students must first be enrolled at their child's home school. See steps 1-5 at the top of this Enrollment and Registration page.
  2. Complete the School Choice Transfer Request Form (PDF).
  3. Complete the Dual Language Intent Form (PDF).
  4. Return both completed forms to the District Office at 3188 Quimby Road. Schools will not accept completed forms.

Completion of an Intent Form does not guarantee your admission to the program. All admission is based on lottery. The first lottery will be on May 30, 2020. A subsequent lottery will be held on June 30, 2020 to fill any remaining spots and develop a waiting list. The waiting list will be maintained should space become available.

Interdistrict Transfer Request

Children residing outside ESD may request enrollment at a school within ESD by first starting the Interdistrict Transfer process at their home school district. Approval must be obtained from the home school district as well as from ESD prior to enrolling at an ESD school. ESD approvals of Interdistrict Transfer Requests are based on space availability for Interdistrict Transfer Requests and require an annual renewal. Parent/guardian is responsible for the transportation of students approved through the Interdistrict Transfer Requests process.
